Introduction

This privacy policy is to provide information to you, my client, on how your personal information (which includes your health information) is collected and used within my practice, and the circumstances in which I may share it with third parties.

Why and when your consent is necessary?

When you register as a client of my practice, you provide consent for  me (Shamarie) to use your personal information so I can provide you with the best possible healthcare. If I need to use your information for anything else, I will seek additional consent from you to do this.

Why do I collect, use, hold and share your personal information?

My practice will need to collect your personal information to provide healthcare services to you. My main purpose for collecting, using, holding your personal information is to manage your health. We also use it for directly related business activities, business marketing and updates from time to time.

What personal information do I collect?

The information I will collect about you includes your:

  • Names, date of birth, addresses, contact details
  • Medical information including medical history, medications, allergies, adverse events, immunisations, social history, family history and risk factors
  • Family and social past experiences
  • Wellness  Indicator Assessment Questionnaire

 

How do we collect your personal information?

My practice may collect your personal information in several different ways.

  1. When you make your first appointment you will be asked to fill in a client intake form online or in person.
  2. During the course of providing Natural Wholistic Health Services, I may collect further personal information.
  3. I may also collect your personal information when you visit my website, send me an email or SMS, telephone me, make an online appointment or communicate with me using social media.

 

When, why and with whom do I share your personal information?

I sometimes share your personal information:

  • with other healthcare providers, when you have given express permission for me to do so
  • when it is required or authorised by law
  • when it is necessary to lessen or prevent a serious threat to a client’s life, health or safety or public health or safety, or it is impractical to obtain the client’s consent.
  • Minimal information such as your name, email and telephone number are also stored as encrypted data by my booking system provider and email provider. These providers are obligated by law to keep your information secure, however, I cannot guarantee their compliance.

My practice will not share personal information with any other third party that is not outlined above without your consent

How do I store and protect your personal information?
My practice stores all personal information securely on a password protected Ipad File.  The backup copies of these files are not stored on line.

Security of Personal Information

Your Personal Information is stored in a manner that reasonably protects it from misuse and loss and from unauthorized access, modification or disclosure.

When your Personal Information is no longer needed for the purpose for which it was obtained, we will take reasonable steps to destroy or permanently de-identify your Personal Information. However, most of the Personal Information is or will be stored in client files which will be kept by us for a minimum of 7 years.

How can you access and correct your personal information at our practice?

You have the right to request access to, and correction of, your personal information. My practice acknowledges clients may request access to their health records. I require you to put this request in writing and my practice will respond within a reasonable time. My practice will take reasonable steps to correct your personal information where the information is not accurate or up to date. From time to time, we will ask you to verify that your personal information held by my practice is correct and current. You may also request that we correct or update your information, and you should make such requests in writing to Shamarie’s Body and Mind Therapies will not charge any fee for your access request, but may charge an administrative fee for providing a copy of your Personal Information.

In order to protect your Personal Information I may require identification from you before releasing the requested information.

Also with regard to cookies etc do we have to have a popup for them the accept that we are collecting information when they visit my website?  I know that at the moment minimal information is collected but I am also aware that through google I can collect more

🌿 About cookie pop-ups

Under Australian Privacy Law, you’re not legally required to use a cookie-consent pop-up unless you actively track or target users for marketing (like with retargeting ads).
However, the EU GDPR and UK require pop-ups, and many Australian sites include them as a best practice.

So you have three options:

  1. Best practice: Add a small, friendly cookie banner (“We use cookies to improve your experience. By continuing, you agree to our Privacy Policy.”).
